Copley Hospital wants to make sure that your medical wishes are carried out.
An Advance Directive is a document that contains specific instructions that guide your physicians and health care providers. It informs them of your treatment preferences in the event you lose the ability to communicate them yourself. The Advance Directive used to be called “A Living Will.”
Our Patient and Family Services staff can offer you information about an Advance Directive. If you wish, they can help you complete a document that becomes part of your medical record.
Contact Patient and Family Services at 888-8311.
More information about Advance Directives are available from the Vermont Ethics Network.
