Benefits |  | Copley Hospital understands that taking care of patients starts with taking care of you and your family. At Copley we offer a wide range of benefits for our employees. Eligibility status is determined by using the guidelines below:
- CLASS I = Employees with authorized hours of 72 or more biweekly
- CLASS II = Employees with authorized hours between 40 and less than 72 hours biweekly
- CLASS III = Employees with authorized hours under 40 hours biweekly
- CLASS IV = Temporary employees
- CLASS V = Per Diem employees who are not regularly scheduled on a biweekly basis

Earned Time Off (ETO): Class I, II, & III Employees. Benefit: 25 paid days off per year; prorated accrual for those with authorized hours less than 80 biweekly; increased accrual after 5, 10, 15 and 25 years. Eligibility: Accrue from date of hire; able to use after 3 months. Paid By: Copley 100%. Medical Insurance (vision included): Class I & II Employees. Benefit: MVP PPO Health Plan. Eligibility: 1st of the month following 1 month of employment. Paid By: Class I: Copley 75%, Employee 25%; Class II: Copley from 50% to 65% depending on FTE status. Dental Insurance: Class I & II Employees. Benefit: Core Plan-Usual & customary plan covering preventative, minor & major restorative and orthodontia. Buy-Up Plan- Plan pays higher % and has a higher calendar year maximum. Eligibility: 1st of the month following 1 month of employment. Paid By: Core Plan: Class I: Copley 100%; Class II: Employee 100%. Buy-Up Plan: Class I & II: Employee 100%. Short Term Disability: Class I & II Employees. Benefit: 60% of weekly earnings to a max of $1,250. Eligibility: 1st of the month following 3 months of employment. Paid By: Employee 100%. Long Term Disability: Class I & II Employees. Benefit: Benefit percentage 60% to a monthly maximum of $6,000. Eligibility: 1st of the month following 3 months of employment. Paid By: Copley 100%. Supplemental Insurance: Class I & II Employees. Benefit: Supplemental insurance benefits available through AFLAC. Eligibility: 1st of the month following 3 months of employment. Paid By: Employee 100%. Flexible Spending Accounts (for medical and dependent care expenses): Class I & II Employees. Benefit: Pre-tax savings for Medical &/or Dependent Care expenses. Eligibility: Upon employment. Paid By: Employee 100%. Retirement Plan: Employees working minimum of 1000 hours per year. Benefit: Copley contributes 4% of base earnings into a MetLife 403(b) account. Eligibility: After 2 years employment working a minimum of 1000 hours each calendar year. Paid By: Copley 100%. Voluntary Retirement Savings: All employees. Benefit: Pre-tax investment options through payroll deduction. Eligibility: Upon employment. Paid By: Employee 100%. Tuition Reimbursement: Class I & II Employees. Benefit: Tuition assistance provided for job-related courses. Eligibility: After 3 months of employment. Paid By: Copley pays a percentage of tuition. Group Automobile and Homeowners Insurance: Class I & II Employees. Benefit: Group rated insurance payable through payroll deduction. Eligibility: After 3 months of employment. Paid By: Employee 100%. Jury Duty: Class I & II Employees. Benefit: Normal wage paid for lost work time. Eligibility: Upon employment. Paid By: Copley 100%. Military Duty: Class I & II Employees. Benefit: Normal wage paid for lost work time up to 2 wks./yr. Eligibility: Upon employment. Paid By: Copley 100%. Bereavement Leave: Class I & II Employees. Benefit: Up to 3 paid days after a death in immediate family. Eligibility: After 3 months of employment. Paid By: Copley 100%. Employee Assistance Program (EAP): All employees.
